Consent Form

We are pleased that donor families entrust us to carry out their donation decisions, and are proud to serve as caring stewards of their gifts. We respect donor families and their wishes by following a process for informed consent.

Even if an individual has signed up through a state donor registry or indicated donation wishes on a drivers license, donation organizations still obtain consent from the family. The consent process includes:

• Explanations on how tissue is recovered, processed, stored and distributed;
  
  
• Explanations that the tissue may be used or modified for transplantation;
  
  
• Explanation that the family may limit or restrict the use of tissue, and
